EFECTIVIDAD DE LOS PROGRAMAS DE EJERCICIO TERAPÉUTICO EN PACIENTES CON DOLOR LUMBAR CRÓNICO: REVISIÓN SISTEMÁTICA Y META-ANÁLISIS

Resumen

El dolor lumbar crónico (DLC) constituye la primera causa de discapacidad a nivel global y una importante carga sanitaria, social y económica para los sistemas de salud. Entre las opciones de tratamientos no farmacológicos, el ejercicio terapéutico se encuentra posicionado por las principales guías de práctica clínica internacionales como un tratamiento de primera línea recomendado, aunque la duda persiste respecto a qué tipo de ejercicio sería el más eficaz. Analizar y sintetizar la evidencia científica del periodo 2016-2026, sobre la eficacia de los programas de ejercicio terapéutico para la disminución del dolor, la discapacidad y la mejora de la calidad de vida en la población adulta con dolor lumbar crónico no específico. Se llevó a cabo una revisión sistemática de la literatura, documental y bibliográfica, con perfil metaanalítico, siguiendo las normas de la declaración PRISMA 2020. Se realizaron búsquedas en PubMed/MEDLINE, en la Cochrane Library, Scopus, Web of Science y PEDro, revisiones de la Cochrane, metaanálisis por red y ensayos clínicos aleatorio de alta calidad metodológica. Se sintetizó evidencia de más de una veintena de estudios de alta calidad. El ejercicio terapéutico, en las distintas modalidades (control motor, pilates, yoga, tai chi, ejercicio acuático y terapias funcionales cognitivas), ha observado clínicamente una reducción relevante del dolor y de la discapacidad en relación a la atención habitual con un tamaño de efecto de moderado a grande, siendo el yoga y el tai chi los tipos de ejercicio, de entre los estudiados, que han mostrado el mayor tamaño de efecto. Los programas breves, de alta frecuencia semanal y biopsicosociales mostraron los mejores resultados a medio y largo plazo. El ejercicio terapéutico representa una intervención segura, efectiva y costo-efectiva para abordar el dolor lumbar crónico, y por lo tanto su prescripción individualizada debería ser uno de los tratamientos clínicos prioritarios a incluir dentro de los programas de rehabilitación.
