READAPTACIÓN FUNCIONAL POST-ACV: INTEGRACIÓN DE PRINCIPIOS DE FISIOTERAPIA DEPORTIVA Y NEUROREHABILITACIÓN BASADA EN EVIDENCIA

El accidente cerebrovascular (ACV) es una de las primeras causas de discapacidad adquirida en adultos, provocando alteraciones motoras, cognitivas y funcionales que limitan la independencia de los pacientes y afectan la calidad de vida de la persona y su familia. Dentro de este contexto, la neurorrehabilitación ha incorporado una serie de principios establecidos en la fizioterapia deportiva: entrenamiento de alta intensidad, fortalecimiento neuromuscular, control motor, propiocepción y readaptación funcional progresiva a fin de optimizar la recuperación funcional. El objetivo de esta revisión fue analizar la evidencia científica publicada entre 2016 y 2026 en relación con la integración de los principios mencionados en programas de rehabilitación funcional para pacientes post-ACV. Se realizó una investigación bibliográfica de enfoque descriptivo-diseño narrativo-sistemático, siguiendo las directrices PRISMA para la identificación, la selección y la síntesis de estudios. La búsqueda se realizó en las bases de datos PubMed/MEDLINE, Scopus, Web of Science, SciELO y Cochrane Library. Los resultados indican que intervenciones tales como entrenamiento aeróbico de alta intensidad, fortalecimiento muscular progresivo, la terapia a partir del uso de restricción del lado sano, la terapia espejo, el entrenamiento orientado a tareas, el entrenamiento neuroatlético y el uso de tecnologías inmersivas (realidad virtual, robótica, tele-rehabilitación) producen mejoras significativas en equilibrio, marcha, fuerza muscular, capacidad cardiorrespiratoria e independencia funcional, siempre y cuando se aplique correctamente el entrenamiento, la progresión y la especificidad del ejercicio. Se concluye que la incorporación de los principios de la fisioterapia deportiva en los protocolos de neurorrehabilitación constituye una estrategia de intervención innovadora, segura y clínicamente relevante para favorecer la recuperación funcional tras el ACV, adoleciendo todavía de la necesidad de estandarizar los protocolos, optimizar la dosificación del ejercicio y desarrollar estudios multicéntricos que respalden la evidencia disponible.
